In addition to the completed and signed Diploma or Certificate Replacement form, be sure to include either the copy of your receipt from our Business Office, or a Cashier’s Check or Money Order in the amount specified below. All University of Illinois graduates with no outstanding University charges will receive one diploma per degree earned, free of charge. They may also submit a written request by mail. Your request may be made in person once the campus reopens, by mail, or by FAX. Butler graduates may order a replacement diploma by completing the Butler University Diploma Order form.
